SUBCONTRACTOR EVALUATION FORM FOR PRIME CONTRACTORS & CONSTRUCTION MANAGERS

PLEASE TAKE THE TIME TO FILL OUT ONE ANNONYMOUS SURVEY FOR EACH OF THE PRIME CONTRACTORS/CONSTRUCTION MANAGERS YOU HAVE PERFORMED WORK FOR OVER THE LAST FIVE YEARS. THIS SURVEY WILL BE USED TO EVALUATE HOW AND IF PRIME CONTRACTORS/ CONSTRUCTION MANAGERS USE BEST PRACTICES WHEN WORKING WITH SUBCONTRACTORS. IT WILL ALSO BE USED TO EVALUATE HOW WELL THESE FIRMS ADHERE TO THE REQUIREMENTS SET FORTH IN SENATE BILL 914. YOUR COOPERATION WOULD GREATLY BE APPRECIATED. Did the Construction Manager (CM)/Prime Contractor (PC) participate in full disclosure for the duration of the project(s)?
Has the CM/PC solicit your company for any private construction projects?
Did the CM/PC offer any alternative financial resources to your company in the form of bonding, Quick Pay, joint check issuance or other viable options?
Did the CM/PC offer any training which would have been pertinent or helpful in the execution of the project such as safety training, LEED training, technical training or any other relevant training? If so, how effective was the training you received?
How would you rate you company’s relationship with the CM/PC after having been awarded the contract?
How proactive was the CM/PC with regards to conflict resolution?
How would you rate negotiations, if any, which occurred during scope review for any and all projects which your company participated in with the CM/PC?
How effective was the CM/PC with regard to overall project management? (i.e., communication with subcontractor, scheduling updates and notifications, meeting notifications, scope change notifications).
Did the CM/PC comply with its regulations and procedures for the site to the extent that they required compliance of those regulations and procedures by subcontractors?
Did the CM/PC break the bid packages up into manageable sizes?
How would you rate the CM/PC with payment of invoices?
How user friendly was the CM/PC with regard to the overall prequalification process?
Did the CM/PC comply with informal bonding limits?
How would you rate the overall performance of the CM/PC?
How would you rate the overall performance of your company when working with the CM/PC?
